GENERAL NATURE OF POSITION:        Provides outreach, engagement, support, role modeling, coaching and/or aftercare for children, adults and/or older adult clients and their families that are participating in our mental health programs. The focus is on assisting the individual to grow and manage their recovery and their families to understand and support their recovery.  This is a part time, 20 hour per week internship position.

 

ESSENTIAL JOB FUNCTIONS:

 

• Assists Case Managers to obtain family member support for participating clients to link them with community treatment services for mental and physical health care, educational programs, financial assistance, part-time employment, housing, advocacy, socialization activities and other services.

·        Provides field peer counseling to clients in reducing their isolation or in communicating effectively with their families.

·        Accompanies clients to needed services

·        Models or coaches families to assist their client to recover.

·        May co-facilitate client and/or family groups.

• Models or coaches clients on appropriate behavior in housing, employment, healthcare, benefit and community situations
• Assists and coaches clients to learn to use public transportation
• Coaches clients seeking employment and supports the activities of the Focus for Work staff.
• Participates in a team environment to provide 24/7 client support.
• Refers and supports client participation in AA, NA, NAMI or other support and educational groups
• May assist other Catholic Charities departments in their respective functions.
• Completes services rendered documents, progress notes and records services as required in client files.
• Completes monthly activity reports.

 

QUALIFICATIONS:

 

Required education and experience:

·        Experience with children, adults or older adults with mental health and/or substance abuse disorders as an individual or as a family member

·        If a family member, participating in family education programs

·        Recovery of mental health and/or substance abuse disorders managed for at least one year

·        Bi-lingual skills required, e.g. Spanish, Vietnamese, Mandarin, and/or American Sign Language, etc.

·        Good oral and written English skills

·        Excellent culturally competent, inter-generational, and interpersonal skills.

·                    Computer skills using Windows

 

Physical Requirements:

· Ability to lift 25 pounds

 

General

·        Valid California driver’s license and personal automobile

·     Liability insurance according to agency requirements
