GENERAL NATURE OF POSITION: Provides case management/brokerage for older adults to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible individuals. Provide basic counseling and crisis intervention services for individuals as needed. Works with family partners to assist the clients in their recovery and management of chronic mental illness, possible substance use and chronic health disorders... In the spirit of the program, “do whatever it takes” to assists the client in accomplishing his/her goals. This is a part time position.

 

ESSENTIAL JOB FUNCTIONS:

  • Intensive case management of 3-4 seriously mentally ill older adults with co-occurring disorders, e.g. substance use, gambling addiction, physical health problems. diagnosed or addicted older adults
  • Coordinates services with psychiatrist, team members, primary care and specialist physicians, legal representatives/guardians, Depts. of Mental Health and Alcohol and Drugs staff, and other community services/organizations to assure the individual’s access to services and the services and the service delivery system.
  • Assists clients in obtaining housing, financial assistance, health care and other community supports.
  • Provides crisis intervention.
  • Assists monolingual clients, as necessary, in their interviews with the psychiatrist.
  • Provides on-call services evenings and weekends to address treatment needs, sobriety issues and short-term crisis situations as appropriate.
  • Conducts outreach activities to identify potential clients in community locations, e.g. correctional facilities, churches, senior centers, community gathering locations.
  • Assists clients in completing applications for services, assistance, ID cards, etc.
  • Connects clients with NAMI and other supportive organizations to assist the clients and their families.
  • Documents all activities according to program requirements.
  • Participates in program audits.
  • Participates in a team environment to provide 24/7 client support.

 

 

QUALIFICATIONS:

 

Required education and experience:

·        Master’s degree preferred in social work, human services, counseling or other behavioral science and 1 year of experience in the field, preferably with mental health and/addicted individuals.

·        Alternative Bachelor’s degree in social work, human services, counseling or other behavioral science and three years experience in the field with mental health and or addicted individuals.

·        Ability to work with diverse populations.

·        Bi-lingual Spanish, Vietnamese, Mandarin or Cantonese preferred

·        Good oral and written English skills.

·        Experience in working with older adults and/or geriatric emphasis in education.

·        If an individual is in recovery, five-six years of sustained recovery is required.

·        Culturally competent communication skills.

 

Physical Requirements:

· Ability to lift 25 pounds
