Description and Services:
The Navigator Project provides case management and service coordination for the chronically homeless in Santa Clara County. Case management and coordination of services includes, but is not limited to, assisting with achieving goals for independence as defined by the person served. Services include:
- Optimizing resources and opportunities through community linkages and enhanced social networks
- Assisting with assessing transportation, personal care, budgeting, medical and/or healthcare services, and linking to other community resources.
Eligibility and Requirements:
- Be chronically homeless according to the following definition: homeless for one year or having had at least four episodes of homelessness in the past three years.
- Have a mental health and/or substance abuse problem/issue.
- Be over 18 years of age.
- Are currently not receiving case management services or actively participating in a treatment program.

Hours and Availability: Monday – Friday, 8:30 a.m. - 5:00 p.m.
