Description and Services:
OASIS specializes in serving adults whose psychiatric issues may be compounded with substance abuse factors and the older adult population. The intent is to assist individuals through their recovery process, and stabilize their symptoms and behaviors so that they can reintegrate back to a less intensive support system. Services include:
- Case management
- Medication support
- Counseling
- Mental health rehabilitation services
Eligibility and Requirements:
- Must be residents of Santa Clara County, age 18 and older
- Services are provided to adults whose serious and persistent mental health issues interfere with their ability to function in the community and without intervention are at risk for higher levels of care (i.e. hospitalization)
Waiting List: Due to the nature of the services individuals are accepted on a space available basis only
Fees:
- Primarily for Medi-Cal recipients
- Some limited spaces for others and the County’s sliding fee schedule is used
Hours and Availability: Office hours Monday-Friday, 9:00 a.m. – 5:00 p.m.
