Description and Services:

We provide Case Management for clients in our programs to assist them in maintaining their housing.  Services include:

• Counseling clients on-site
• Assessing client needs and resources, creating action plans, and evaluating outcomes
• Helping clients find resources and access services they need
• Organizing community-building activities for residents of apartment buildings

Eligibility and Requirements: Formerly homeless individuals or families who found housing through housing services of Catholic Charities of Santa Clara County

Hours and Availability: Monday – Friday, 9 a.m. - 5:00 p.m.  Walk-ins are welcome.
